FAFSA Verification Forms

SPECIAL NOTE: Beginning July 13, 2021 and for the remainder of the 2021-22 FAFSA processing and verification cycle (including summer periods attached to the 2021-22 award year), the U.S. Department of Education (ED) is waiving verification of FAFSA information, except for identity/Statement of Educational Purpose and high school completion status in Verification Tracking Groups V4 and V5. However, conflicting information on FAFSAs still must be resolved. MBU may still be contacting you for Verification items if we see conflicting information on your FAFSA. 

Verification is a process of reviewing specific information from your FAFSA. The government generally selects the applications and the specific information within those applications that schools must review.

Sometimes we will select an application on its own if there is reason to believe the information from your FAFSA is incorrect or if it conflicts with other information we receive. If additional information is required, the instructions and additional forms needed will be provided to you.

On this page, you will find forms for each type of information you are required to verify. Within the form, additional information might be requested, such as tax information. You will also find guides and instructions to help you collect or fill out the forms you provide.

Deadline for submitting documentation: Documents requested for verification must be submitted before your aid will be disbursed, and no later than 2 weeks before the end of a semester. You may lose your eligibility for Title IV funds if you fail to submit requested documents by the stated deadline.
